feat(prefetch): async expert prefetcher v3.2 for the OLMoE testbed Testbed-only scope: olmoe.c + tools/oracle files; glm.c untouched (the production engine already ships the equivalent techniques: coalesced slab preads, PILOT lookahead, persistent PIN).
