fix: rename remaining teacher/student refs, remove .gradio from repo

- Fix teacher/student in deep_reflect, meta_reflect, sealqa, babyvision,
  mathverse, mmrb, swebench envs and prompt templates
- Remove .gradio/certificate.pem from tracked files
- Add .gradio/ to .gitignore
